How this consent form uses your information
Why we ask
The Inkpot uses the information in this form to prepare for your tattoo, identify matters the artist should discuss with you, record your consent and maintain an appropriate appointment record.
Health information
Health information receives additional protection. The form asks for your explicit permission to process the listed health information for tattoo safety. A Yes answer is reviewed by an artist; it is not used by an automated system to accept or reject your appointment.
Where it is kept
Your answers, signature and completed PDF are encrypted in the dedicated Inkpot consent service. They are not stored in WordPress, Telegram, Facebook or Instagram messages, Google Calendar, or the InkpotAI conversation history.
Who can see it
Access is limited to authorised Inkpot staff who need the information for your appointment or record management. Service providers acting for The Inkpot may process encrypted data under appropriate contractual and security controls.
Photography
Photography permission is a separate choice. Declining it does not affect your booking or tattoo.
Retention and your rights
Completed consent records are retained for two years after the appointment, after which the record and encrypted files are scheduled for deletion. Unsubmitted forms are scheduled for deletion within 30 days of cancellation or expiry. You can contact The Inkpot to ask about your information, correct inaccurate details, withdraw health-data consent before the tattoo takes place, or exercise your applicable data-protection rights. Some rights may be limited where the studio must retain a record to establish, exercise or defend legal claims.
